Hermes’ surgery is scheduled for next week, and a discussion is needed this afternoon.

This surgery is a major one, in which a large portion of the tibia will be discarded, leaving only the healthy parts at both ends and removing all the infected bone tissue in the middle.

Then, using Ilizarov’s technique, an external fixation frame will be used to lengthen the healthy bone tissue at both ends, extending from both ends towards the middle until they converge.

To accomplish bone elongation, an external fixation frame has to be fitted to the calf. With a screw on it, the frame will move along with the bone tissue as the screw is turned.

The elongation process will begin two weeks after the surgery, and extend by one millimeter each day. Hermes will need an extension of over thirty centimeters, which would take approximately a year.

This is just the ideal situation. If any abnormalities occur in the bone formation process midway, the elongation will have to be halted.

In simple terms, it’s using slow and continous traction to get the tissue back to its fetal period growth stage. This technology has revolutionised traditional pressure bone healing theory.”

Yang Ping deeply respects the pioneers of these medical theories. These people have big imaginations and rich imaginations.

Professor Zhang added on to the origin of the theory: “Ilizarov was a rural doctor in Siberia in the Soviet Union during the 1950s and 60s. This technique was the result of an accident. The initial purpose of his external fixator design was to treat a fracture patient. Every day, by turning the screw on the external fixator, the fractured ends could be tightly compressed, which would promote fracture healing. Once, when he was on leave, he instructed a nurse to adjust the screw on the external fixator daily. The nurse was very serious and acted strictly according to his medical advice; the only mistake she made was turning the screw in the wrong direction. By the time Ilizarov came back, the patient’s fractured ends had been separated, but surprisingly, new bone callus had formed. Inspired by this phenomenon, he invested a lot of time and effort into research, eventually proposing the Tension Stress Rule, or the Theory of Traction Tissue Regeneration. The Soviet Union then invested a lot of manpower and resources into research, perfecting this theory, resulting in the mature theory we have now. This theory is fully deserving of the Nobel Prize, but at the time of the US-Soviet Cold War, naturally it did not win. China began implementing this technique in the 1980s. There was a time when this technique was abused to lengthen normal people to increase their height; it caused many complications such as skin necrosis, muscle weakness, non-union of bone, nerve injury, and so on. In October, 2006, the Ministry of Health ordered a ban on osteotomy to increase height for ‘medical cosmetic treatment’, that is, it could not be used to increase the height of normal people. It can only be used for congenital deformities, trauma, infection, tumors, and other reasons for bone defects and unequal limb length, as well as limb deformities caused by disease.”

The discussion not only made the patient’s diagnosis and treatment plan clearer and more comprehensive, but it was also a great opportunity for young doctors to learn. Through clinical cases, experience is accumulated, the knowledge of a category of diseases is acquired with each typical case.

After such a discussion, everyone had a basic understanding of Ilizarov’s magical technique.
